We’re devoted to ensuring complete transparency in our pricing for our patients. That means you won’t encounter co-pays, hidden fees for standard medical procedures, or surprise medical bills. Additionally, we offer discounted rates for lab tests and necessary medical supplies.

Membership Pricing

$160/month

for an individual
We love to see families, therefore we offer the following discounts:

Couples

$300/month maximum

Families

$400/month maximum

(2 adults and up to 3 children)
Single Parents

$160 for adult, $50 per additional child

Please note we charge a one-time enrollment fee of $100 per person. If you are an existing patient of Dr. Wax’s this will be waived.

Including pap smears, IUD* placement, and contraceptive counseling
*cost of IUD not included but may be covered by your insurance or available at discounted rate

On-time appointments and extended length visits

30-90 minute visits, no waiting room

Office visits when you need them. Plus, direct access to your doctor via phone, text, or email.

Telehealth

All included in flat monthly fee

After Hours Availability

Call, email, text your needs and we will get back to you in a timely manner
*we will review how to best utilize our after hours services prior to enrollment

What's Not Covered?

  • In-hospital care; although I will be available to consult with hospitalists when indicated
  • Emergency Care
  • Obstetrical services
  • Chronic pain management requiring or involving the use of controlled substances
  • Services that are most appropriately referred to specialists
  • DOT physicals
  • Worker’s compensation claims/disability evaluations/auto accident forms
  • Imaging services
